Üel Life and the clinicians on our platform are required by law to:

How we may use and disclose PHI
For treatment
To provide, coordinate, or manage your healthcare. Example: sharing your prescription with our partner pharmacy.
For payment
To obtain payment for services. Example: confirming with Square that your card was charged.
For healthcare operations
To support business activities of our healthcare providers. Example: quality improvement, training.
As required by law
When disclosure is required by federal, state, or local law, including public health reporting.
To business associates
We share PHI with vendors who provide services on our behalf, under Business Associate Agreements that require them to safeguard your information.
Uses requiring your authorization
The following require your written authorization:
- Marketing communications about products from third parties
- Sale of PHI
- Most uses and disclosures of psychotherapy notes
Your rights
You have the right to:
- Inspect and copy your PHI. Request via info@uel.life.
- Request amendment of PHI you believe is incorrect.
- Receive an accounting of disclosures we have made.
- Request restrictions on certain uses and disclosures.
- Request confidential communications (contact only at certain numbers or addresses).
- Receive a paper copy of this notice.
- File a complaint without retaliation.
